Confidentiality


CelebraMED LLC  takes safeguarding the client’s information extremely seriously. CelebraMED LLC is bound by law to protect client confidentiality. Any disclosure of your treatment to others will require your explicit written consent. However, CelebraMED LLC providers are mandated reporters.If indications of child abuse, elder abuse, or planned bodily harm to oneself or to others come to light in the course of treatment, the providers are legally bound to report these to the appropriate authorities. Clients are required to sign a statement of understanding of their rights to privacy, which will be provided via Patient Portal.

Medication Refills

Medications will not be refilled outside of business hours including nights and weekends.
You must allow 48-72 hours for refills to be processed. It is your responsibility to request a refill in a timely manner.
We have the right to refuse refills at any time


Payment

Payment for services is due at the time of scheduling. Appointments canceled with at least 48 business-hours’ notice will receive a full refund of the prepayment. The prepayment will not be refunded for canceled appointments with less than 48 business hours’ notice or missed/no-show appointments. Clients must pay with a credit card.
If payments are 60 days past due, CelebraMED LLC reserves the right to utilize legal resources such as collection agencies or small claims court to obtain payment for our services.


Ryan Haight Act

Providers may not prescribe controlled substances without an in-person examination at least once per year, per the Ryan Haight Act. Therefore, CelebraMED LLC requires one (or as recommended by the provider) in-person examination per year if a controlled substance prescription is required.

​**Please note that we take a conservative approach in prescribing a class of medications called Benzodiazepines for anxiety disorders.  These medications include: Ativan, Xanax, Klonopin, Valium. If you are on these medications, we will be working on a taper and moving towards stopping. We are happy to discuss alternative medications.
